CARITAS HOSPICE SERVICES

  • Nursing Staff

    We have a great team of licensed practical nurses and registered nurses that will walk you through this journey. They are on call and can help with any type of crisis day or night.

  • Medical Supplies

    Medical supplies are provided to the patient through our hospice. Such as personal care products including soap, shampoo, body lotion, and barrier creams. Briefs, pads, and other incontinence care supplies

  • Continuous Care

    This round-the-clock level of care is provided in the home for brief periods of time when the patient is experiencing a crisis. It is designed to honor a patient’s wish to remain at home by providing the care needed to control the patient’s symptoms and alleviate the crisis.

  • Medications

    When joining Caritas, our team will streamline the medication process. We strive to improve the safe and quality use of medicines when patients transfer between the care they are currently on and the transition to our hospice.

  • Certified Nursing Assistants

    Certified nursing assistants are an entry-level member of a patient's healthcare team, performing important patient-centered tasks under the supervision of licensed nursing staff. They help with ambulation, grooming, and routine daily activities.

  • Social Services

    Social services can lead support groups and provide individual, couples, and family counseling. They also provide in-service training to other professionals involved in patient care on topics like Compassion Fatigue, End-of-Life Decision Making and Planning, and Coping with Grief.

  • Spiritual Services

    Spiritual services are an important part of hospice care—the interdisciplinary, compassionate care services that support patients and families through the end-of-life and grieving process.

  • Physician Coordination

    Changing to hospice can be scary. We are here to make your doctor's transition easier. We will coordinate with your past doctor, to get the best care from our doctor on staff.

  • Respite Services

    Respite care is short-term relief for in-home hospice caregivers. Medicare benefits pay for patient transport and up to five consecutive days of inpatient care at a Medicare-approved nursing facility or hospital.

  • Distinctive Care Plan

    We strive to elevate the end-of-life experience in very unique ways. Expect highly attentive, hands-on care. We are always there for you before, during, and after for hospice care.

  • Veteran Recognition

    We recognize that caring for the men and women with military service is an honor and important responsibility. Military veterans may have life experiences that require a unique approach to hospice

  • Dementia Acknowledgement

    Caritas now has a certified facilitator that can provide a glimpse of daily living for a loved one or resident living with dementia. Call today to request a tour for your facility

  • Volunteering

    Volunteering is important here at Caritas. We offer all different kinds of opportunities to volunteer. Such as companionship, visitation, quilting group, craft group, and honor a vet. Please read and use our volunteer application in the drop-down menu, under contact us.

  • Durable Medical Equipment (DME)

    There are only a couple of hospice medical equipment providers that all hospice companies use. While they do a good job, in certain situations, an emergency can arise where equipment is needed faster. When this does, we can provide our own equipment at a moment’s notice to assist with hospital discharges for emergencies.
