Hospice General Criteria Guide
If your loved one is showing two or more of the following signs, it may be time to begin a conversation about hospice care. Speak with your physician to explore a care plan that best supports comfort, dignity, and quality of life.
Possible Indicators That Hospice May Be Appropriate:
Increased need for help with daily activities (bathing, dressing, eating, mobility)
Unintentional or ongoing weight loss
Noticeable changes in mental clarity or alertness
Frequent or repeated falls
Skin breakdown or slow-healing wounds
Frequent changes to medication or dosage
Worsening shortness of breath or fatigue
Multiple emergency room visits or hospitalizations
Recurring infections
Symptoms becoming increasingly difficult to control
Treatments no longer providing benefit, or
A personal choice to stop aggressive or curative interventions
Other chronic or serious conditions contributing to overall decline
