Hospice General Criteria Guide

If your loved one is showing two or more of the following signs, it may be time to begin a conversation about hospice care. Speak with your physician to explore a care plan that best supports comfort, dignity, and quality of life.

Possible Indicators That Hospice May Be Appropriate:

  • Increased need for help with daily activities (bathing, dressing, eating, mobility)

  • Unintentional or ongoing weight loss

  • Noticeable changes in mental clarity or alertness

  • Frequent or repeated falls

  • Skin breakdown or slow-healing wounds

  • Frequent changes to medication or dosage

  • Worsening shortness of breath or fatigue

  • Multiple emergency room visits or hospitalizations

  • Recurring infections

  • Symptoms becoming increasingly difficult to control

  • Treatments no longer providing benefit, or

  • A personal choice to stop aggressive or curative interventions

  • Other chronic or serious conditions contributing to overall decline

  • Cancer

  • Pulmonary Disease

  • Dementia / Alzheimer’s

  • Stroke / Neuro

  • Renal Disease

  • Liver Disease
